Is the rotation permanent?

Yes. The new orientation is saved in the file, so it opens the right way in every viewer.

Does rotating reduce quality?

No. Rotation only changes how the page is displayed; the content itself is untouched.

Can I rotate a password-protected PDF?

Yes. Type the password when asked; it is used only on your device. The rotated PDF is saved without a password. Use Protect PDF afterwards if you want one again.
